Case 10b: A 19-year-old man from China with severe transfusion-dependent aplastic anemia underwent cord transplantation from an incompletely matched donor. His conditioning regimen included antithymocyte globulin, cyclophosphamide, and total body irradiation. On day 47 after transplant, his bone marrow had engrafted, but he presented to the transplant clinic with dysuria and hematuria for 2 weeks. He reported passing blood and clots in the urine, but he had no fevers, chills, or back pain. His immunosuppressive medications included cyclosporine and mycophenolate mofetil.
