The evaluation of abdominal pain in the recent traveler is discussed in this chapter. Concerning symptoms include fever, blood or mucous in the diarrhea, and severe abdominal pain. Many causes of diarrhea can be diagnosed clinically and managed symptomatically, but others such as parasitic or enteroinvasive bacterial causes are managed with antiparasitic or antibiotic medications in addition to hydration. This chapter highlights a patient with crampy, intermittent abdominal pain; diarrhea; and flatulence. He has recently been white-water rafting and used chlorine tablets to purify his drinking water. The author reviews exam and testing, diagnosis of Giardia, treatment, and subsequent confirmation of the diagnosis by stool sample.
