This case discusses the risk factors and classification for aortic dissection, as well as diagnosis and management in the emergency department setting. It reviews the 2 classification structures, DeBakey and Stanford systems, commonly used for aortic dissections. The symptoms of aortic dissection can be variable, and therefore a wide differential must be considered. The authors discuss how to use D-dimer to rule out aortic dissection when the suspicion is low but pursuing further diagnostic testing when the suspicion is high. Management of aortic dissection in the emergency department setting involves aggressive blood pressure and heart rate management and immediate consultation with surgical expertise for definitive medical or surgical management.
